3. (school district) Enrollment Packet – Complete entire document & return

Completed enrollment documents can be returned to your student’s school office or emailed to brsafterschool@gmail.com

After submitting your completed enrollment packet, please await a phone call from the program coordinator at your student’s school. You and the program coordinator will review your enrollment documents together, answer any questions, and establish a start date for your student. In the meantime, should you need assistance, you can you call the Children’s Services Department of Blue River Services, Inc. at 812-364-1142.
